Oral Supplements - Carnitine
Carnitine is a health supplement that is seen in high levels in the epididymis (the small organ behind the testicle that serves as a “refinery” for the sperm where last minute tune-ups and touch-ups are done on the sperm before they head out to your ejaculate). It has been suggested that it helps in protecting sperm from toxic Reactive Oxygen Species and helps sperm move better by helping the sperm engines (mitochondrias) run more efficiently.
Unfortunately, the utility of carnitine oral supplements is surrounded with controversial and conflicting data. Preliminary evidence suggests that low levels of carnitine in the semen are associated with male infertility. Some studies have found that oral supplementation of carnitine may help improve sperm motility; however, these studies were poorly controlled. More recent studies that were better designed and controlled suggest that there is no added benefit for taking the oral carnitine supplements (Sigman et al. Fertility and Sterility 85:1409, 2006).
If you are interested in discussing the use of carnitine, please feel free to bring it up to Dr. Kuang. Carnitine supplements can be taken with minimal side effects. While it has not been convincingly shown to have a positive effect on male fertility, it may be a viable option for you and your partner if it meets your reproductive needs and timetable. Dr. Kuang is more than happy to help you navigate making the best decision for you as a couple.
